Names and functions of devices and appliances

Mouthpiece (aligner) :

The mouthpiece is a custom-made, transparent, mouthpiece-type orthodontic appliance. They are worn on the teeth and are replaced periodically to gradually shift their position and align the teeth. The KiyoClear Orthodontics treatment uses two types of mouthpieces, soft and hard, for each stage. Please follow the directions from the KiyoClear-aliated clinic for the duration of the wearing period and daily wearing time.

Expansion plate (expansion device)

This device is used to solve the lack of space for the teeth to align. Depending on your condition, your dentist may recommend using this device in combination. It creates space by expanding the dentition buccally and uses the space to align the front teeth esthetically. If you are using this device, please be sure to bring it with you every time.

※Use of expansion plate is not a part of the treatment by KiyoClear Orthodontics but is an additional treatment by the aliated clinic at the discretion of the dentist. Please contact the aliated clinic for details.

Tips for attaching and detaching devices

Mouthpiece: How to attach it?

  • Step 1 : Place the mouthpiece on the front teeth and t it along the alignment of the teeth from the front to the back.

  • Step 2 : Press down firmly with your fingers to x it in place. Please be careful not to bite down too hard on the mouthpiece, as this may cause it to damage. If using a chewie, use it after pressing the mouthpiece rmly into place with your fingers.

※If you apply too much force, it may cause deformation. Please be careful not to apply too much force in any direction when attaching or detaching the device.

Mouthpiece: How to detach it?

  • Step 1 : Upper jaw: Place your index finger or the hook of the remover on the rim of the inner part of tooth no. 7 or 8 and push down. Using your right hand on the left back tooth and your left hand on the right back tooth will make it easier to remove it. Lower jaw: Place your thumb or the thin part of the remover on the rim of the mouthpiece on the cheek side of tooth no. 7 or 8 and push up.
  • Step 2 : Place your fingers around the tooth no. 2 and no. 3 on both sides of the front teeth and remove the entire mouthpiece.

※To ensure smooth removal, KiyoClear has shortened the height of the mouthpiece for the posterior teeth.

Expansion plate: How to attach it?

  • Make sure the direction is correct and place the device in the mouth. Push the plate straight upward for the upper jaw (and straight downward for the lower jaw) until it fits into the upper wall of the oral cavity.

※Use of expansion plate is not a part of the treatment by Kirei Lign Orthodontics but is an additional treatment by the aliated clinic at the discretion of the dentist. Please contact the aliated clinic for details.

Expansion plate: How to detach it?

  • Upper jaw: Place your finger or the hook of the remover in the recess of the expansion plate at the front side of tooth no. 4 or the back side of tooth no. 7 (circled in red) and pulled down to remove.

  • Lower jaw: Place your fingers or the thin part of the remover in the recess of the expansion plate at the back of tooth no. 7 (circled in red) and pulled up to remove.

Method and duration of use
  • Mouthpiece

Wear it for at least 20 hours per day. If you need to remove it for meals or other reasons, it is recommended to remove it for no more than an hour at a time and for as short a time as possible. This is because if you remove it for more than an hour, your teeth may return to their original position beyond the extent that can be recovered by reattaching it. For example, if you absolutely cannot avoid removing your mouthpiece for 2 hours, wearing it for just 5 minutes in the middle of the period, such as an hour later, will help prevent your teeth from moving back to their original position. If you remove it for more than 4 hours a day, you may experience a delay in your treatment schedule.

  • Expansion plate (expansion device)

    • Insert the screw into the upper threaded hole and turn it in the direction of the arrow (↓) until it stops at 90 degrees. This is one turn.

※Note that each turn is 90 degrees, so be careful not to turn 360 degrees.

    • Turn before bedtime or at other times when you can wear it continuously for the longest period of time.

    • Wear it for a total of at least 8 hours per day. It is fine to divide the time into several times instead of 8 hours continuously at one time. However, we recommend that you wear the device for at least 30 minutes at a time. If you wear the device for a shorter period of time per day or per time, you may not achieve the desired treatment effect.

※ Use of expansion plate is not a part of the treatment by KiyoClear Orthodontics but is an additional treatment by the aliated clinic at the discretion of the dentist. Please contact the aliated clinic for details.

4
I don't know when to replace the mouthpiece

When to replace the mouthpiece depends on which stage of treatment you are in and whether you use an expansion plate. Please use this page only as a reference and contact the KiyoClear-aliated clinic you are visiting for more information.

In order to retain the dentition after the teeth are moved, it is necessary to wear a retainer. After using your last hard-type mouthpiece for at least 2 weeks, please visit the clinic to have a mold of your teeth taken for making your retainer. Be sure to continue using the hard-type mouthpiece until you receive your retainer.

5
Can I replace my mouthpiece earlier than scheduled?

Shortening the wearing period than the one scheduled is like “pulling your hair because you want them to grow faster.” If you ignore the growth cycle of your hair and pull it, it will eventually fall out. There is a risk of accelerating the mechanism of discoloration, devitalization, pain, mobility, and loss of teeth due to root resorption, alveolar bone resorption, gingival recession, and pulp damage. Therefore, please adhere to the wearing period specified by the KiyoClear-aliated clinic. If you are using an expansion plate in conjunction with a mouthpiece, please follow the instructions given by the aliated clinic regarding the duration of wearing the mouthpiece.

6
Mouthpiece is cracked or punctured

Bruxism or chewing too hard can cause the mouthpiece to wear down and become punctured. A few cracks and holes will not adversely affect your teeth, and if they are around the posterior teeth, they will hardly interfere with the treatment. However, cracks across the six anterior teeth may affect the pressure on the teeth and may require replacement. Please contact the dentist of the KiyoClear-aliated clinic you are visiting as soon as possible. If you apply too much force, it may cause deformation, so please be careful when you attach or detach it. If teeth grinding is a contributing factor, a mouthpiece to guard against teeth grinding can be used in conjunction with the KiyoClear Orthodontics mouthpiece. If this is your preference, please consult with your dentist at the Kirei Lign-aliated clinic.

7
The expansion plate is cracked or broken

If you are able to wear it stably, it is safe to continue using it. If it is not stable, please contact the KiyoClear-aliated clinic you are visiting.

8
The device does not fit tightly or comes off easily
  • Please contact the clinic you are visiting for checking the condition of your mouth and adjusting the device.

  • If a device is damaged or lost, it can be replaced

※This information is current as of January 2022, and is subject to change without notice.

9
I ’m worried that my mouthpiece is not tight enough

Feeling tightness and pain from the mouthpiece are not indications of your teeth moving. If the force is strong enough to cause persistent pain, it will rather temporarily stop the teeth from moving, causing resorption and damage to the tooth root and alveolar bone. Although teeth movement can be observed afterward, once resorption occurs, no regeneration is possible. The teeth will move most quickly and safely when a continuous, low force is applied. Therefore, even if you do not feel any tightness, please do not worry and continue with your treatment. However, if you have any concerns, please contact the KiyoClear-aliated clinic you are attending.

10
Can I eat and drink while wearing the mouthpiece?

We advise you to remove the mouthpiece during meals to prevent damage and discoloration on the mouthpiece. We recommend you drink water cooler than 40 degrees or unsweetened sparkling water when wearing the mouthpiece.

※Not allowed

  • Beverages that contain sugar or citric acid can cause cavities.
  • Water above 40 degrees can cause deformation of the mouthpiece.
  • Water above 40 degrees can cause deformation of the mouthpiece.

11
My teeth feel loose

Since the teeth move during treatment, depending on the condition of the patient's teeth, some looseness may be felt, and loose movement of about 1 mm is not uncommon. However, it is unlikely that vertical movement will occur during treatment, so if you feel the tooth is moving vertically, another factor such as periodontal diseases may be the cause. Please contact the KiyoClear-aliated clinic where you are undergoing treatment as soon as possible.

12
Will my teeth relapse after treatment is completed?

If you do not use the retainer correctly, the teeth will regress even after the treatment is completed, not just in the case of KiyoCLear Orthodontics. After treatment is completed, we recommend the use of a retainer for at least 2 years as a retention period. After the rst year of wearing the retainer, the daily wear time is gradually reduced, but it is recommended to wear the retainer for as long as possible because the teeth will continue to move for a lifetime.

13
What is a retainer (retention device)?

After treatment is completed, the alignment of the teeth needs to be stabilized. If left unattended after orthodontic treatment, not just with KiyoClear Orthodontics, the teeth tend to shift back to their original position. A retainer is a device that holds the dentition in place to prevent this from happening. We recommend that you wear the retainer at least 20 hours per day for at least 1 year, then gradually reduce the amount of time you wear it, and after the second year, wear it only at night.

※The schedule for use varies depending on the individual. Consult with your dentist for details.
